Had this same problem back in October truck would miss shows code 128 SID 5 Fmi 5. cut truck off and restart and might go 200,500 or 700 miles before it does it again. Had replace engine harness a year earlier. Replaced # 5 injector and has ran about 30,000 miles since with no problems. Trip this week about 2000 miles did it one time same code. 2nd trip about 500 miles done it about 4 times returning today already done it 4 times in 200 miles. I know they had terminals problems and found the harness we replaced about 15 months ago and the date on the bag it came in said 10-12-2012 I would think it would be new style. any Ideals?

Driver just called in and it doing it every 25-35 miles now he said when it does it he can just clear the code out while driving and it quits missing and ok for a while.

It could still be the connector end. You can buy the complete injector connector and soldered it into the harness. You got to make sure all the oil is cleaned from the wire to get a good solder. You can also just get the 4 terminal ends too. This would be the cheapest thing to try. Was the harness secured correctly under the valve cover--not rubbing on any valve springs? Have you checked the injector harness under the valve cover--Is it brittle?

3. Engine ECU If Problem still exists after numbers 1 and 2.

Injector harness is not brittle, bought 4 new terminals and installed this morning. Found a pin that fit tight in new terminals but was loose in the plug on #5, truck leaving for Florida tonight will see what happens, do you know the part # on the complete soldered on harness? Funny you can clear the code without stopping and it quits missing.
