Back around maybe 01 or 02 DD had a batch of bad pistons to get in some engines. The crown or top of the piston would debond from the frame of the piston or the part that goes around the wrist pin.If the driver in most cases shut it down when it happened it would not be a total lose.But many would drive on to the next exit and some times the top of the piston would wedge in the top of the liner side ways and the rod will go some were else.Most all other engines had same problem to some degree. I think the C12 cat was more often.

Snub it off by putting it in high gear and let the clutch out while holding the brake.

When they run away, you can't get it into any gear. Had a friend loose a motor in an Rd a few years ago, could not get it into any gear. Ended up losing the motor. The only hope you have is to take away the air, block the air cleaner.

don't know about the 4 stroke d.d. but the 2 stroke would accelerate at 2 grand / sec. so you almost have 2 seconds to get a phone book (bigun) over tubo/blower, coveralls clipboard what ever. So if any thing to snuff the intake is more than 2 seconds away from the intake, run for your life.......you will have lots of time to spread the floor dry after a bref 4-6 second last screaming of the demon :tease:

this was unfortunately a common event on the 4cyl version, the series 50 with the balance shafts in the oilpan..... we were losing them at about 700,000km on our Orion highway coaches... left part numbers and oil/coolant down our local highways... dot had words with the management and they started to pull at 650km...

on teardown it was discovered in many cases it was #2 piston seizing and there were some known issues about cracks developing in the crank which eventually gave way. Crazy to see an ejected conrod complete with the crank segment still attached and correctly torqued. Worse one I saw had the complete block and crank at #2 cleaned off, block integrity front to rear was maintained by the head, should have taken a photo...
