It's chain drive, has a tag axle, a Buda diesel and what looks to be sheet metal from an E model. I figured it was custom built back when industrious people without any money did such things but after consulting with my Mack books it may have been a factory build. You are correct on the cab being the same as an E model. I would say that is a pretty rare truck. It was definitely a factory model, but the third axle may have been added afterwards. It wouldn't me surprise me if someone wanted to give it a new home. Doesn't look like Mack Lanova it's worth to point out. Great find, very rare as it seems to me. Actually there are not many sources to check out the correct look. There are a few (one or two?) factory photo's in F-series Mack archive book. And those represent typical E-model fenders. The same style as on Gary Mahan's truck. Looking the chassis number you posted 1010D it's 10th truck made. So possibly special fenders were used. Or just aftermarket convertion (but of quite good quality).
